Job summary

Qualifications

• Current license to practice as a physical therapy assistant.
• Graduate from an accredited physical therapy assistant program (CAPTE).
• One year PTA home health experience preferred or approval by regional leaders for less experience.
• Current CPR certification.

Responsibilities

• Provide physical therapy services under supervision and according to plan of care.
• Observe, record and report patient responses and condition changes to supervising PT and staff.
• Documentation of supervisory activities and patient progress in medical records.
• Promote wellness and prevention and coordinate follow-up care with other providers.
• Maintain confidentiality of patient information and participate in quality improvement activities.
• Perform other duties as assigned.

Education
