Granite Peak Home Health in Missoula, MT is seeking a licensed physical therapist to join our growing team. The PT will provide physician-prescribed therapy per plan of care and help restore patients to their highest level of function.

This role emphasizes collaboration with the Home Health Director and PTs, adherence to agency policies, and participation in QAPI and educational activities, with a Monday–Friday schedule.

Qualifikationen

• Graduate from a two-year college-level program approved by the American Physical Therapy Association.
• Licensed, registered, or certified in the state(s) where practicing.
• Two years of experience preferred.

Aufgaben

• Provides physician-prescribed physical therapy under a plan of care established by the PT.
• Improves or minimizes residual physical disabilities of the patient.
• Returns the individual to optimum and productive level within the patient's capabilities.
• Participates with other health care personnel in patient care planning.
• Performs services planned, delegated, and supervised by the PT.
• Instruct patients and family in home programs and activities of daily living.
• Participates in in-service programs and presents in-service programs as assigned.
• Participates in QAPI activities as assigned.
