Job summary

Granite Peak Home Health is seeking a dedicated Physical Therapist to join our Montana team in delivering physician-prescribed therapy in patients' homes.

You will assess, plan, and reassess care, collaborate with the care team, and ensure HIPAA compliance and accurate documentation. A Montana PT license and BLS/CPR are required, with preferred home health experience. Flexible schedule and supportive environment offered.

Qualifications

• Graduate from an accredited physical therapy program.
• Currently licensed to practice in Montana.
• 1 year home health experience preferred.
• BLS/CPR certification required.
• Knowledge of HIPAA compliance and patient privacy requirements.
• Knowledge of local healthcare market and community outreach strategies.

Responsibilities

• Understands and adheres to established policies and procedures.
• Provides physician-prescribed physical therapy.
• Maintains plan of care with 30-day reassessments.
• Initiates and completes OASIS assessments and required documentation.
• Collaborates with home care team in patient care planning.
• Directs and supervises personnel as required.
• Writes progress notes to physicians about patient progress.
• Instructs patients and families in home programs and activities of daily living.
• Attends patient care conferences and supports QA/QI activities.
