Job summary

Granite Peak Home Health seeks a licensed physical therapist to provide physician-prescribed therapy in patients' homes, following a PT-approved plan of care. This role emphasizes improving patient function, collaborating with the care team, and educating patients and families.

The position reports to the Home Health Director, with a hybrid work arrangement in Montana. Prior home health experience and HIPAA compliance knowledge are preferred, and eligibility to practice in the state is required.

Qualifications

• Graduate from a two-year college-level program approved by the American Physical Therapy Association.
• Licensed, registered, or certified in the state(s) in which practicing.
• Two (2) years experience, preferred.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Provides physician-prescribed physical therapy under a plan of care established by the PT.
• Improves or minimizes residual physical disabilities of the patient.
• Returns the individual to optimum and productive level within the patient’s capabilities.
• Participates with all other health care personnel in patient care planning.
• Performs services planned, delegated, and supervised by the PT.
• Instruct patients and family members in home programs and activities of daily living.
• Prepares medical records, and progress notes and updates care plans for each patient visit promptly as per Agency policy.
• Participates in educating the patient and family.
