Job summary

Granite Peak Home Health in Great Falls, MT seeks a dedicated Physical Therapist to provide physician-prescribed therapy at patients' homes. You will conduct initial assessments, develop care plans, and work with a multidisciplinary team to restore function and independence.

Requirements include MT licensure, PT degree, BLS/CPR, and familiarity with HIPAA regulations. A clean driving record and 1 year home health experience are preferred. Mileage reimbursement and flexible scheduling are offered.

Qualifications

• Graduate from a physical therapist curriculum approved educational institution.
• Licensed to practice in Montana.
• 1 year home health experience preferred.
• BLS/CPR required for this position.
• Knowledge of HIPAA, patient privacy and healthcare regulations.

Responsibilities

• Provide physician-prescribed physical therapy.
• Improve or minimize residual physical disabilities.
• Return patient to optimum functioning within capabilities.
• Participate in patient care planning with other home care personnel.
• Direct and supervise personnel as required.
• Complete initial assessment and Plan of Care with quarterly reassessments.
