Job summary

Granite Peak Home Health in Butte, MT is seeking a licensed Physical Therapist to provide physician-prescribed PT in patients' homes. The role focuses on improving function and independence through client-centered plans of care.

You will collaborate with a care team, document progress, and educate patients and families on home exercise programs. A two-year APTA-approved program and state licensure are required.

Qualifications

• Graduate from a two-year APTA-approved program.
• Licensed, registered, or certified in the state of practice.
• Ability to exercise initiative and independent judgment.
• Familiarity with HIPAA and patient privacy requirements.

Responsibilities

• Provide physician-prescribed PT under a plan of care.
• Improve or minimize residual disabilities.
• Return patient to optimum level within capabilities.
• Collaborate with health care team in care planning.
• Document care plans and progress promptly.
• Educate patients and family in home programs.
