Job summary
Granite Peak Home Health in Montana is seeking a licensed Physical Therapist to join our growing team. You will provide physician-prescribed PT services in patients' homes, assess function, and develop plans of care.
The role requires Montana licensure, BLS/CPR, HIPAA compliance, and the ability to work with families; a driving license is required. We offer mileage reimbursement and a flexible schedule.
Qualifications
- Graduate from a physical therapist curriculum approved educational institution.
- Currently licensed in the state of Montana.
- 1 year home health experience, preferred.
- BLS/CPR required for this position.
- Knowledge of HIPAA compliance and patient privacy requirements.
- Knowledge of local healthcare market and community outreach.
- Knowledge of software including WellSky (EHR).
- Clean driver’s license with a clean driving record.
Responsibilities
- Provides physician-prescribed physical therapy.
- Develops and maintains plan of care with regular reassessments every 30 days.
- Directs and supervises personnel as required.
- Communicates progress to physicians with reports.
- Assists with patient and family education and home programs.
- Participates in Quality Assurance activities and patient care conferences.
