The Christ Homecare is seeking a Physical Therapist Assistant I (PTA I) to provide skilled home health services under the supervision of a Physical Therapist. You will educate patients and families, document thoroughly, and ensure safety and compliance with Medicare/federal guidelines.

Join our team to support patients’ independence, coordinate with clinicians, and participate in on-call and in-service trainings. A valid PTA license and driver’s license are required.

Qualifications

• Minimum of one year of PTA experience in an acute setting.
• Home care experience is desired.
• Ability to effectively communicate with patients, families, physicians and coworkers.
• Knowledgeable on therapy best practices.
• Ability to make appropriate physical therapy judgments.
• Detail-oriented and observant.
• Disciplined work ethic with the ability to prioritize and be timely.
• Valid driver’s license and auto insurance in the driver's name.

Responsabilités

• Provides skilled services in a patient’s home as developed by the Physical Therapist plan of care.
• Observes, records, and reports patient reactions, changes, and progress.
• Performs physical therapy procedures and diagnostic tests as prescribed.
• Treats patients to relieve pain, develop or restore function, and maintain maximum performance.
• Assesses functional problems and works with the therapist to achieve functional goals.
• Communicates with physician, therapist, and coworkers; participates in case conferencing.
• Recommends devices for patients and educates staff and patients on therapy modalities.
• Documents all treatment, assessments, and patient education; completes chart audits as requested.
