Job summary

The Christ Homecare in Cincinnati is seeking a Physical Therapist Assistant I (PTA I) to deliver skilled services in patients' homes under a Physical Therapist plan of care. You will educate patients and families, ensure safety, and complete documentation in line with Medicare/Federal guidelines.

The PTA I collaborates with physicians and care teams, observes patient progress, and contributes to goal-oriented therapy interventions, while adhering to state practice scope and agency protocols.

Qualifications

• Minimum of one year PTA experience in an acute setting.
• Home care experience preferred.
• Excellent communication with patients, families, physicians and coworkers.
• Valid driver's license and auto insurance in your name.

Responsibilities

• Provides skilled PTA services in a patient's home as directed by the Physical Therapist plan of care.
• Observes, records, and reports patient progress and reactions.
• Documents all aspects of treatment and patient education in a timely manner.
• Communicates with physician, therapist, and care team; participates in conferences.
• Drives to patient's primary location per scheduled visit and attends assigned visits.
