Job summary

ABL Health Care in Milpitas, California is seeking a Full Time Physical Therapist (PT) to join our Home Health team. You will evaluate patients, create care plans, and monitor progress in patients' homes, working with our Clinical Director and staff.

The role requires an active California PT license, at least 1 year in Home Health or Hospice, and a valid CA driver’s license. Willingness to travel within a 15–25 mile radius is expected.

Qualifications

• Current PT License to practice in California - Required.
• Experience in Home Health or Hospice care - 1 Year Required.
• Current CA Driver's License.
• Able to travel within a 15 - 25 mile radius on a regular basis.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient's home.
• Regularly evaluates and re-evaluates patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Initiates, develops, implements and revises the patient's plan of care (POC).
• Addresses all problems identified in the POC.
• Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition.
• Communicates with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.
