Job summary

Sobe Innovative Rehab is seeking a compassionate Physical Therapist Assistant to provide geriatric home-visit therapy in Leesburg, FL. This PRN role offers autonomy and the chance to contribute to residents’ independence while fitting visits around your availability and other commitments.

Responsibilities include delivering skilled PT according to the care plan, documenting promptly, and communicating with physicians and families to ensure continuity of care. Active Florida PTA license required.

Qualifications

• Associate degree from an accredited PTA program.
• Active Florida PTA license.
• Two years of clinical experience preferred (not required).

Responsibilities

• Provide skilled physical therapy treatments under the plan of care.
• Help patients improve mobility, function, and independence.
• Communicate effectively with physicians, families, and the care team.
• Complete accurate, timely documentation.
