Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist Assistant (PTA) to provide geriatric therapy during home visits in Lakeland, FL. This PRN role supports residents in Assisted Living Facilities and private homes, delivering care aligned with the plan of care and promoting independence.

The successful candidate holds an Associate degree from an accredited PTA program, Florida PTA licensure, and has the flexibility to accept visits in the 33805 and 33801 ZIP codes.

Qualifications

• Associate degree from an accredited Physical Therapist Assistant program.
• Active Florida PTA license.
• Two years of clinical experience preferred (not required).
• Able to see patients in 33805 and 33801.

Responsibilities

• Provide skilled physical therapy treatments under the plan of care
• Help patients improve mobility, function, and independence
• Communicate effectively with physicians, families, and the care team
