Job summary

CenterWell Home Health is seeking a dedicated Physical Therapist Assistant to deliver care in patients’ homes under the supervising Physical Therapist. You will implement the treatment plan, document care, and support functional improvements for geriatric patients.

Requirements include a current PTA license, valid driver’s license with auto insurance, CPR certification, teamwork, and fluent English. Preferred: 1 year of home health experience. TB screening is part of Humana policy.

Qualifications

• Current state license to practice Physical Therapy
• Valid driver’s license, auto insurance and reliable transportation
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Comprehensive knowledge of changes in the geriatric population
• Read, write, and speak fluent English

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in patients' homes, as developed by the Physical Therapist
• Administer non-complex manual therapeutic exercises, therapeutic massage, and modalities
• Document client records and submit clinical documentation on time each week
• Attend educational meetings and coordinate department in-services
• Notify Physical Therapist or Branch Director of equipment issues or staff changes
• Apply infection control and universal precautions in performing patient care
