CenterWell Home Health, part of Humana Inc., is seeking a Home Health Physical Therapist Assistant to support patients in their homes under the guidance of a Physical Therapist. You will implement therapy plans, perform therapeutic exercises and modalities, and document care while coordinating with the clinical team.

Requirements include an active state PT license, CPR, a valid driver's license, and strong English communication. Prior experience with geriatric patients and teamwork is preferred.

Qualifications

• Active state Physical Therapy license (PT/ PTA as applicable).
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Experience collaborating with a healthcare team.
• Knowledge of geriatric physical changes and aging.
• Proficiency in English (speaking, reading, writing).

Responsabilités

• Administer the physical therapy treatment plan to patients in their homes as developed by the PT.
• Perform non-complex manual therapy and related modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Report equipment failures or staffing or client changes to supervisor.
• Apply infection control and universal precautions in patient care.
• Make a measurable impact on patient outcomes.
