Job summary

CenterWell Home Health, part of Humana Inc., is seeking a Therapy Assistant to administer PT care in patients' homes under the supervision of a Physical Therapist.

The Home Health PTA provides treatment, documents records, participates in in-services, and ensures infection control while delivering compassionate care to geriatric patients.

A valid PTA license, CPR, driver's license, and English proficiency are required; preferred one year of experience.

Qualifications

• Current state PTA license to practice Physical Therapy.
• CPR certification is required.
• Valid driver's license and reliable transportation are required.
• Experience collaborating with a team of healthcare professionals.
• Knowledge of geriatric population changes, medically and socially.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes, as developed by the Physical Therapist.
• Perform non-complex active and passive manual therapies and modalities as ordered.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify supervisor of equipment failures or changes in staff or clients.
• Apply infection control and universal precautions in patient care.
