Job summary

OhioHealth in Columbus seeks a Physical Therapy Assistant to provide one-on-one patient care in the home health setting. You will work with an interdisciplinary team, enjoy a flexible schedule, and participate in a fair weekend rotation while maintaining independence in care.

In this role you will assist in assessments and plans of care, educate patients and families, coordinate discharge planning, and report progress to the supervising PT.

Qualifications

• Associate's degree is required and must be licensed.
• BLS certification required; Ohio PTA license.
• Experience in home health or inpatient rehab preferred.
• Strong interpersonal and teamwork skills.

Responsibilities

• Direct patient care in home health following physician referral.
• Assist in assessment and contribute to plan of care.
• Support discharge planning with multidisciplinary team.
• Educate patients and caregivers/family.
• Document evaluation and progress with PT supervision.
