Job summary

PurposeCare is seeking a licensed Physical Therapist Assistant to join our home health team in Illinois. You will provide skilled visits in patients’ homes, assess progress, and adjust therapy plans to improve daily living and mobility.

Flexible scheduling and autonomy are offered, with emphasis on compassionate, one-on-one care for clients in their own homes. Experience in home health is preferred, and a valid driver’s license is required.

Qualifications

• Current Physical Therapist Assistant license in the state you are applying
• 1 year of PTA experience preferred - home health experience preferred
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
• Current Driver's License, vehicle insurance, and/or access to dependable transportation
• Ability to successfully complete a background check

Responsibilities

• Provide skilled PTA visits in patients' homes
• Assess patient needs and progress during home visits
• Develop and adjust treatment plans within home care setting
