PurposeCare is seeking a Physical Therapy Assistant to join our home health team in the Midwest region, delivering skilled visits in patients' homes. You will work with patients to improve mobility and daily living activities, guided by individualized treatment plans and ongoing evaluation.

Qualifications

• Current Physical Therapist Assistant license in the applying state.
• 1 year PTA experience preferred; home health experience preferred.
• Ability to perform physical activity including lifting up to 50 lbs.
• Valid driver's license, vehicle insurance, or reliable transportation.
• Background check completion.

Responsibilities

• Provide skilled physical therapy visits in patients' homes.
