Job summary

TheraCare is seeking a licensed Physical Therapist Assistant (PTA) for a full-time home health position serving adult and geriatric patients in Willow Brook, TX and surrounding areas. The PTA will provide one-on-one in-home physical therapy with a flexible daytime schedule and clinical autonomy.

Qualifications include an accredited PTA degree, Texas PTA license, 1 year of Home Health experience, CPR, and a valid driver’s license. HCHB experience is preferred. Comprehensive benefits are offered.

Qualifications

• Associate degree from an accredited PTA program.
• Active Texas PTA license.
• Home Health experience (1 year) required.
• CPR certification current.
• HCHB experience preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled home health physical therapy interventions under the supervision of a licensed PT.
• Implement and carry out individualized Plans of Care (POC) set by supervising PT.
• Treat orthopedic, neurological, cardiac, and post-surgical patients.
• Administer therapeutic exercises, gait, balance, and mobility training.
• Apply manual therapy and non-complex modalities per physician orders.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Monitor patient response and report changes to the supervising PT.
• Document accurately per Medicare home health regulations.
• Follow infection control and home health safety protocols.
• Participate in QAPI and agency performance improvement initiatives.
