Job summary

Discovery at Home in Florida is seeking a licensed Physical Therapy Assistant to join our care team. You will support the physical therapy plan, assist patients with exercises and ADLs, and help maintain progress documentation. The role requires home health experience, Medicare/Medicaid knowledge, and reliable transportation.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Licensed by the State as a Physical Therapy Assistant.
• One year of home health experience required.
• Knowledge of Medicare and Medicaid guidelines.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skills; ability to use Microsoft Windows.

Responsibilities

• Assist the PT in planning and implementing patient care and therapy services.
• Assist patients with exercises, mobility, and ADLs as directed by the PT.
• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries.
• Maintain accurate and timely documentation of patient progress.
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care.
• Provide home health and/or outpatient services as needed to ensure optimal outcomes.
