Best Home Health Providers, Inc. is seeking a Physical Therapist Assistant to work under a licensed PT, delivering hands-on therapy in patients' homes across the Bay Area. The role emphasizes mobility restoration and independent activities, with per diem pay of $95 per visit.

Qualified candidates hold an APTA-accredited PTA degree, California PTA license, CPR certification, and a valid driver’s license with reliable vehicle. Travel to patient homes is required.

Qualifications

• Associate's degree from an APTA-accredited PTA program.
• Active PTA license from the California Physical Therapy Board.
• Current CPR certification with AHA.
• Strong communication and organizational skills.
• Valid driver’s license with reliable, insured vehicle and willingness to travel to patient homes.

Responsibilities

• Deliver hands-on physical therapy under the direction of a licensed Physical Therapist, following each patient's care plan.
• Guide patients through therapeutic exercises, gait and transfer training, and the use of assistive devices (wheelchairs, braces, crutches, canes, prosthetics).
• Apply physical modalities including heat, cold, ultrasound, electrical stimulation, and massage to relieve pain and restore function.
• Monitor patient progress and communicate changes to the Physical Therapist and care team.
• Document care accurately and on time, and participate in team meetings and ongoing training.
• Perform other duties as required for patient services
