Job summary

Best Home Health Providers Inc. is seeking a Physical Therapist Assistant to work with a licensed Physical Therapist in patients' homes across the Bay Area. This per diem role offers meaningful hands-on PT, patient mobility support, and home-based recovery guidance.

Responsibilities include exercises, gait training, assistive devices, and partnering with the care team to monitor progress. Must hold an CA PTA license, AS PTA degree, CPR (AHA), and a valid driver's license.

Qualifications

• Associate's degree from an APTA-accredited PTA program.
• Active PTA license in California.
• Current CPR certification (AHA).
• Strong communication and organizational skills.

Responsibilities

• Deliver hands-on physical therapy under the direction of a licensed Physical Therapist, following each patient's care plan.
• Guide patients through therapeutic exercises, gait and transfer training, and the use of assistive devices.
• Apply modalities including heat, cold, ultrasound, electrical stimulation, and massage to relieve pain and restore function.
• Monitor patient progress and communicate changes to the Physical Therapist and care team.
• Document care accurately and on time, and participate in team meetings and ongoing training.
• Perform other duties as required for patient services
