Qualifications

• Current California PTA license to practice.
• At least 1 year of Home Health care experience.
• Valid California driver’s license.

Responsibilities

• Provide PTA services within the patient’s plan of care and coordinate with the Interdisciplinary Team.
• Monitor and document patient progress; report changes to the Physician/Case Manager as needed.
• Travel to patient homes within a 15–25 mile radius and collaborate with clinical staff.
