Job summary

ABL Home Health in Oakley, CA is hiring a Physical Therapist Assistant (PTA) for a full-time role in home health. You will evaluate patients, develop individualized treatment plans, and work to improve mobility and quality of life, in collaboration with our clinical team.

Qualifications

• Valid California PTA license required.
• 1 year experience in home health.
• Must have CA driver's license and ability to travel within 15–25 miles.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms indicating a change in patient condition to Physician, Case Manager, and/or Director of Nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
• Comply with all company policies and procedures to ensure high standards of care.
