Job summary

ABL Health Care is seeking a dedicated Physical Therapist Assistant (PTA) for full-time home health in Mountain View, CA. You will evaluate patients, develop individualized treatment plans, and help improve mobility and quality of life while coordinating with the clinical team.

Qualifications

• Licensed Physical Therapist Assistant in California required.
• Minimum 1 year of Home Health experience.
• Valid California driver’s license and ability to travel 15–25 miles regularly.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to the Physician, Case Manager, and/or Director of Nursing as needed.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.
