Job summary

SunCrest Home Health, part of the LHC Group and within the Optum family, is seeking a Physical Therapist Assistant. The role focuses on delivering therapeutic exercise, patient education, and care coordination under supervision to improve patient well-being in home settings.

You will adhere to state laws, support plan-of-care changes with the supervising PT, and educate families and care teams. Weekend availability is required and driving to visits may be necessary.

Qualifications

• Current, licensed, registered, or certified to assist in physical therapy in the state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.

Responsibilities

• Follow all state-specific laws governing home care PT; follow plan of care defined by supervising PT; suggest changes to plan as needed.
• Instruct and assist patients with active and passive exercise, gait, ADL, and mobility training.
• Observe, record, and report patient responses; coordinate care with the healthcare team.
• Educate patient, family, caregiver, and other health team members on therapy within PTA scope.
• Function effectively in diverse home settings.
