Job summary

SunCrest Home Health, part of the LHC Group and UnitedHealth Group family, seeks a Per Diem Physical Therapist Assistant to deliver therapy per the supervising PT’s plan. You will educate patients and families, perform therapeutic exercises, and coordinate with the care team to enhance patient well-being.

Requirements include active PTA licensure, a valid driver’s license, and weekend availability; CPR certification is preferred or achievable within 90 days.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• Current CPR certification or ability to complete within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility.

Responsibilities

• Follow state-specific laws governing provision of physical therapy in home care and adhere to the supervising PT’s plan of care.
• Instruct and aid patients in active and passive exercise, gait, ADL, and transfer training.
• Observe, record, and report patient responses; coordinate care with the healthcare team.
