Job summary

SunCrest Home Health, part of LHC Group and under UnitedHealth Group, seeks a Per Diem Physical Therapist Assistant to support home-based care and patient education. You will implement the plan of care, guide exercises, and monitor progress in a home-health setting.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation
• Available to work a weekend rotation.

Responsibilities

• Follow state laws and treatment plan as defined by supervising PT
• Instruct and aid patients in active and passive exercise, gait, ADL, and mobility training
• Observe and report patient responses; coordinate care with health team
• Educate patient, family, and staff on therapy within scope of practice
• Function in diverse home settings and treat all patients with respect and dignity
