Graham Healthcare Group is hiring a Home Health Physical Therapy Assistant to serve patients in Elmhurst, Illinois. The role is 40 hours per week with an 8-hour shift and staff employment type, offering a competitive hourly rate.

The PTA will collaborate on patient care plans, drive to patient locations, and use EMR software to document care. A licensed PTA with a valid driver's license and one year of relevant experience is preferred.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Minimum one year of experience in an acute care or rehabilitative setting preferred.
• Current license/registration as a Physical Therapist Assistant in the state of practice.
• Valid driver’s license and ability to drive up to 20% of the day.

Responsibilities

• Collaborate to provide management and delivery of patient care plans.
• Support patients in home health settings in Elmhurst and surrounding areas.
• Maintain timely charting and documentation to meet deadlines.
• Participate in after-hours care as part of the on-call team.
