Job summary

Graham Healthcare Group is seeking a Home Health Physical Therapy Assistant to support patients in Proviso, Illinois and surrounding areas. The role offers a 40-hour work week, 8-hour shifts, and a staff employment type with a $5,000 first-year bonus.

Qualified candidates hold an Associate degree in Physical Therapy, a current PTA license, and a driver's license with the ability to drive to patient locations. EMR experience and strong time-management are required.

Qualifications

• Must have an associate degree in physical therapy from an accredited college or technical school.
• Must hold, in good standing, a license/registration as a Physical Therapist Assistant in the state you work.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to meet critical deadlines for charting.
• Experience with tablets, mobile phones and EMR software.

Responsibilities

• Collaborate to provide management and delivery of patient care plans.
• Support patients in their home health program and coordinate with the care team.
• Contribute to documentation and communication to ensure quality care.
