Graham Healthcare Group is seeking a Home Health Physical Therapy Assistant for a 40-hour-per-week role in Darien, Illinois. The position offers a pay range of $25 to $40 per hour and supports patients in Downers Grove and surrounding areas.

Our team-based environment emphasizes collaboration to manage and deliver patient care plans using EMR software, tablets and mobile devices. This role requires an active PTA license and a current driver’s license with travel to patient locations.

Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Minimum one year of acute care or rehabilitative experience preferred; home health experience preferred.
• Must hold a license/registration as a Physical Therapist Assistant in the state of work.
• Current driver’s license and ability to drive ~20% of day to patient locations.
• Commitment to meeting deadlines for charting.

Responsibilities

• Collaborate to manage and deliver patient care plans as part of a team.
• Support patients in their home settings across Darien, Downers Grove and surrounding areas.
• Document care using EMR software and mobile devices.
• Provide compassionate, timely and accurate patient care in accordance with plan of care.
