Job summary

UnitedHealth Group is seeking a Physical Therapist to join the Home Health team in the DFW area. You will perform assessments, determine plans of care, and deliver interventions to improve mobility and function for patients in their homes.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs and assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and the plan of care, in coordination with the health care team.
• Observes and reports the patient's response to treatment and changes in condition to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patients, families, and care team regarding plan of care, functional limitations, and progress toward goals.
• Visits patients per Plan of Treatment, completes progress notes per policy.
• Completes all evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care; function in diverse home settings.
