Job summary

Qualifications

• Current Physical Therapy licensure in state of practice.
• Valid driver’s license and vehicle insurance or access to reliable transportation.
• Preferred CPR certification or ability to obtain within 90 days.

Responsibilities

• Evaluates patient function, mobility, and therapy needs.
• Develops and implements the plan of care in collaboration with physician and care team.
• Provides interventions to improve function and reduce pain.
• Educates patient and family on plan of care and progress toward goals.
• Documents progress notes after each visit and reports outcomes.
