Job summary

Residential Home Health and Hospice is seeking a licensed Physical Therapist Assistant to deliver skilled care in patients' homes within the Marion, Carbondale, and surrounding areas. The role supports mobility, pain reduction, and functional independence under supervision of a licensed PT, with a strong emphasis on patient education and safe device use.

You will travel to patient homes, maintain thorough EMR documentation, and collaborate with a care team.

Qualifications

• Active Physical Therapist Assistant license in the state you work.
• Minimum 1 year of PTA experience delivering patient care.
• Valid driver’s license and ability to drive ~20% of the workday.
• Home health experience preferred.

Responsibilities

• Provide compassionate, patient-centered Physical Therapy services in the home health setting under the direction of a licensed Physical Therapist.
• Support patients in restoring mobility, reducing pain, and maximizing functional independence through therapeutic exercises, gait training, transfer training, muscle re-education, and prosthetic or orthotic support.
• Educate patients and caregivers on the safe use of mobility and assistive devices, monitor progress and communicate patient responses to the care team.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.

PTA Qualifications

• Active Physical Therapist Assistant, PTA license in the state you work

• One year of experience as a Physical Therapist Assistant and the clinical expertise to deliver quality patient care

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
