Job summary

Residential Home Health and Hospice in Naperville seeks a dedicated Physical Therapist Assistant (PTA) to provide skilled home health care. You will work under a licensed PT, helping patients regain mobility, reduce pain, and achieve independence through targeted therapy and home safety training.

Requirements include an active PTA license, at least 1 year PTA experience, and a valid driver’s license with ~20% travel to patient homes.

Responsibilities

• Provide compassionate, patient-centered PTA services in the home health setting under the direction of a licensed Physical Therapist.
• Assist patients to restore mobility, reduce pain, and maximize functional independence through exercises, gait training, and mobility training.
• Educate patients and caregivers on safe use of mobility devices and monitor progress and communicate changes to the care team.
• Maintain thorough EMR documentation and work independently within a supportive team.

What You’ll Do:

• Provide compassionate, patient-centered Physical Therapy services in the home health setting under the direction of a licensed Physical Therapist.
• Support patients in restoring mobility, reducing pain, and maximizing functional independence through therapeutic exercises, gait training, transfer training, muscle re-education, and prosthetic or orthotic support.
• Educate patients and caregivers on the safe use of mobility and assistive devices, while monitoring progress and communicating patient responses and condition changes to the care team.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

PTA Qualifications

• Active Physical Therapist Assistant, PTA license in the state you work
• One year of experience as a Physical Therapist Assistant and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
