Qualifications

• Degree in physical therapy from an accredited program.
• Two years PT experience; home health preferred.
• Good verbal and written communication and organization skills.
• CPR certification and a valid driver's license.

Responsibilities

• Provides physical therapy services per physician's plan of care and state acts.
• Assess and develop total plan of care and evaluate home environment.
• Directs therapy, teaches patients and families, and documents progress.
• Instructs caregivers and may train on prosthetic devices when needed.
• Supervises PT assistants and coordinates with home health staff.
