Job Juncture in Colorado is seeking a dedicated Physical Therapist to join our home health team. This full-time position focuses on delivering patient-centered therapy in patients' homes, promoting mobility and independence.

You will perform assessments, design personalized treatment plans, guide exercises, educate families, and collaborate with a multidisciplinary team while maintaining compliant records. Requirements include CAPTE-accredited Master’s in Physical Therapy, current state

Qualifications

Minimum nine months experience as a physical therapist.
Preferred community/home health experience.
CAPTE-accredited Master’s degree in Physical Therapy.
Active state licensure to practice as a physical therapist.
CPR certification required; maintain current CPR.
Valid driver’s license to travel to all locations.

Responsibilities

Perform comprehensive physical assessments and analyze medical histories to evaluate patients’ conditions.
Create and implement tailored treatment plans addressing rehabilitation goals.
Conduct therapeutic exercises and training programs to improve mobility and functionality.
Educate patients and families about injury prevention, wellness strategies and recovery.
Monitor progress and adjust treatment plans to achieve rehabilitation goals.
Collaborate with a multidisciplinary healthcare team to ensure holistic patient care.
Maintain meticulous patient records and documentation in compliance with standards.
