Job summary

Human Touch Home Care is seeking a dedicated Physical Therapist Assistant to join our Colorado team, delivering patient-centered home health care. You will work under a supervising PT to implement care plans, educate patients and families, and document progress.

Applicants should hold a CAPTE-accredited PTA program and Colorado PTA license or be eligible, with preferred experience. This full-time role offers competitive pay and a supportive team environment.

Qualifications

• Completion of a PTA program accredited by CAPTE or equivalent.
• Current PTA license in Colorado or eligible to practice.
• Two years' experience preferred.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Performs services planned, delegated and supervised by the Physical Therapist.
• Assists in preparing clinical and progress notes.
• Educates the patient and family.
• Prepares medical records and updates care plans for each patient visit.
• Maintains patient confidentiality and safeguards information.
• Reports changes in patient condition to the supervising PT.
