Optimal Home Care is seeking a dedicated Physical Therapist to provide patient-centered home health services in Colorado. You will evaluate and treat patients, develop and document plans of care, educate families, and coordinate discharge planning while adhering to HIPAA and professional standards.

Qualifications

• Requires active Colorado license to practice as a Physical Therapist.
• Prefers at least one year of clinical experience as a Physical Therapist.
• Must demonstrate effective written and verbal communication and strong interpersonal skills.

Responsibilities

• Complies with ethical and professional standards per APTA guidelines.
• Ensures patient safety and HIPAA compliance during care.
• Evaluates patients and assists physicians with the Plan of Care.
• Develops and documents Plan of Care and treatment frequencies.
• Treats patients to relieve pain and restore function.
• Coordinates referrals for out-patient services when needed.
• Documents progress notes weekly and educates patients and families on use of assistive devices.
• Plans for discharge and prepares summaries and instructions.
• Participates in staff development and emergency preparedness activities.
