Job summary

Choice Health at Home is seeking a Home Health Physical Therapist to deliver one-on-one care in patients' homes in the Denver area. The role offers autonomy, meaningful patient relationships, and a path to impact beyond the clinic.

The successful candidate will evaluate and treat patients, develop individualized plans, and collaborate with a diverse care team while maintaining compliant documentation and safety standards.

Qualifications

• Graduation from an accredited Physical Therapy program.
• CPR certification is required.
• Valid driver’s license and reliable transportation.
• Home health experience preferred.
• Homecare Homebase (HCHB) experience is a plus.
• Strong communication, organization, and time-management skills.

Responsibilities

• Perform comprehensive initial evaluations and identify patient needs, functional limitations, and goals.
• Develop and implement individualized plans of care to improve mobility, strength, balance, and function.
• Provide skilled physical therapy interventions in patients’ homes.
• Evaluate patient progress and adjust treatment plans to optimize outcomes.
• Educate patients and caregivers on exercises, mobility techniques, fall prevention, safety, and ongoing progress.
• Collaborate with physicians, nurses, therapists, caregivers, and the interdisciplinary team.
