Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide patient-centered care in a home-based setting. You will evaluate impairments, design therapy plans, and implement interventions to improve mobility and daily living skills under physician orders and Branch Director guidance.

This role requires a valid state license, reliable transportation, and an ability to work in diverse home environments while meeting company standards for quality and safety.

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation in good working condition.
• Must be able to drive in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Help patients improve mobility and independence in activities of daily living.
• Administer skilled care per physician orders under the direction of the Branch Director.
