Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide skilled services in patients' homes. You will evaluate mobility, develop treatment plans, and implement interventions that improve independence and function.

The role requires driving to patient locations, maintaining professional standards, and collaborating with a multi-disciplinary team to support high-quality home-based care.

Qualifications

• Must possess a valid state driver’s license.
• Automobile liability insurance is required.
• Seatbelts must be worn at all times while driving.
• Reliable transportation kept in good working condition.
• Must be able to drive in all weather conditions.
• Licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments to improve mobility and daily living activities.
• Deliver skilled physical therapy services per physician orders and plan of care.
• Educate patients and families; coordinate with the care team and Branch Director.
