Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide skilled care to homebound patients under physician orders. The role focuses on improving mobility, independence in self-care, and functional skills through evidence-based physical therapy modalities.

Responsibilities include teaching families and care teams, coordinating with the branch director, and adhering to established plans of care.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Physical therapists examine and treat patients with physical impairments using modalities.
• Help patients improve mobility and daily living skills.
• Deliver services under physician orders and care plans.
• Work under supervision of the Branch Director.
