Job summary

BAYADA Home Health Care is seeking a Full-Time Admissions Physical Therapist (PT) to join our Rhode Island home health visit team. You will complete OASIS SOCs for adult and geriatric patients across RI, collaborating with a multidisciplinary team to deliver skilled nursing and rehabilitative care at home.

Responsibilities include home visits, PT evaluations, care planning, and education for clients and families.

Qualifications

• Current Rhode Island Physical Therapist License.
• Prior Home Health and OASIS experience strongly preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, balance, gait, strength, endurance).
• Develop and implement individualized care plans including manual therapy and gait training.
• Continually assess and revise the PT care plan with nursing in the team-based plan.
• Educate clients and families on rehabilitative care to promote independence.
• Accurately document observations and interventions using a touch‑pad tablet.
