Job summary

BAYADA Home Health Care is seeking a Full-Time Admissions Physical Therapist to join our Rhode Island home health visit team. You will perform OASIS SOC for adult and geriatric patients and collaborate with a multidisciplinary team to deliver skilled nursing and rehabilitative care in the home.

Responsibilities include making visits, evaluating mobility and safety, developing individualized care plans, and educating clients and families.

Qualifications

• Must have current Rhode Island Physical Therapist license.
• Prior Home Health and OASIS experience strongly preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, balance, gait, strength, endurance).
• Develop and implement individualized care plans including manual therapy, gait training and balance interventions.
• Continually assess and revise the PT care plan and participate with nursing in the multidisciplinary care plan.
• Educate and instruct clients and family members in rehabilitative care and activities to promote health and independence.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.
