Job summary

BAYADA Home Health Care is seeking a Full-Time Admissions Registered Nurse (RN) to join our home health visit team in Westerly, Rhode Island. You will complete OASIS SOCs for adult and geriatric patients and work as part of a multi-disciplinary team.

One year of RN experience is required; Home Health and OASIS experience are strongly preferred. BAYADA emphasizes compassionate, reliable care delivered in the home setting.

Qualifications

• Current Rhode Island nursing license in good standing.
• At least 1 year of hands-on medical experience with adults/geriatric patients.
• Medicare certified home health and OASIS experience preferred.

Responsibilities

• Complete OASIS Starts of Care (SOC) for adult and geriatric patients in southern Rhode Island.
• Triage and manage urgent clinical situations in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Conduct comprehensive wound assessment, care, and documentation.
