Job summary

BAYADA Home Health Care is seeking a Full-Time Admissions Physical Therapist to join our Rhode Island home health visit team. You will perform OASIS SOC for adult and geriatric patients and collaborate with a multidisciplinary team to deliver skilled nursing and rehabilitative care in the home.

Responsibilities include making visits, evaluating mobility and safety, developing individualized care plans, and educating clients and families.

Qualifications

• Must have current Rhode Island Physical Therapist license.
• Prior Home Health and OASIS experience strongly preferred.

• Educate and instruct clients and family members in rehabilitative care and activities to promote health and independence.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.

BAYADA Home Health Care is currently seeking a Full-Time Admissions Physical Therapist (PT) to join our home health visit team. You will complete OASIS Starts of Care (SOC) for adult and geriatric patients throughout Rhode Island. Home Health and OASIS experience is strongly preferred. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. We believe our clients and their families deserve the highest quality home health care delivered with compassion, excellence, and reliability.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a touch pad tablet.

Qualifications for a Physical Therapist include:

• A current Rhode Island Physical Therapist License.
• Prior Home Health and OASIS experience strongly preferred
