BAYADA Home Health Care, Inc. seeks a Full-Time Admissions Physical Therapist (PT) to join our Rhode Island home health visit team. You will perform OASIS Starts of Care for adults and seniors, traveling to patient homes and collaborating with a multidisciplinary team to deliver skilled nursing and rehab services.

The role emphasizes building care plans, educating clients and families, and documenting care on a touch pad tablet in a supportive, values-driven organization with strong benefits.

Qualificações

• Active Rhode Island Physical Therapist license.
• Home health and OASIS experience preferred.
• Ability to travel to patient homes within designated territory.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements relevant to PT care.
• Develop and implement individualized care plans and interventions.
• Continually assess and revise the PT care plan with nursing and team.
• Educate clients and families to promote health, safety and independence.
• Document observations and interventions in electronic records.
