Job summary

BAYADA Home Health Care is seeking an experienced Admissions Physical Therapist (PT) for a full-time role delivering OASIS Start of Care visits to adult and geriatric clients across Monmouth, Middlesex, Union, and Somerset Counties, NJ. This position offers autonomy in a home health setting and requires strong clinical assessment and care coordination.

Qualifications

• New Jersey Physical Therapist license required.
• Minimum of one year of PT experience.
• Home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination.
• Excellent communication and time management.

Responsibilities

• Complete comprehensive OASIS Start of Care admissions visits for home health clients in the assigned territory.
• Perform comprehensive PT evaluations of mobility, strength, balance, gait, endurance, transfers, safety and functional abilities.
• Assess home environment for safety and barriers to independence.
• Develop individualized plans of care based on client needs and physician orders.
• Establish goals and recommend ongoing therapy services.
• Coordinate nursing, therapy, social work, and other home health services as needed.
• Educate clients, families, and caregivers on mobility, safety, exercises, and care plan.
• Collaborate with physicians and the interdisciplinary team to coordinate care.
• Ensure timely OASIS assessments and admission documentation.
• Document evaluations and care coordination in BAYADA EMR.
