Job summary

Advocate Health seeks a Physical Therapist (PT) for home health case management in the Chicago southeast area. You will deliver patient-centered PT services in patients' homes, coordinate care with families, and support regulatory compliance.

The role requires a Master’s in Physical Therapy, IL licensure, BLS, and ability to travel. A sign-on bonus up to 10,000 is available, along with comprehensive benefits and incentive pay.

Qualifications

• Master's Degree in Physical Therapy required.
• Licensed in the State of Illinois (or eligible for licensure) required.
• Basic Life Support (BLS) for Healthcare Providers certification (AHA) to be obtained within 6 months unless exempted by leadership.

Responsibilities

• Provide home health PT services delivering quality patient care and safety.
• Coordinate care with families and the care team using SBAR framework.
• Document thoroughly to support reimbursement and regulatory requirements.
• Lead or participate in interdisciplinary case conferences as appropriate.
• Assist with discharge planning and referrals to ensure seamless transitions of care.
